Mobile case management for current and former foster youth aged 14-25

             WHY IS THIS PROGRAM VITAL FOR TRANSITIONING FOSTER YOUTH?

  • Less than half of all foster youth in California graduate high school

  • 75% of young women in foster care report at least one pregnancy by age 21

  • Every year about 4,000 youth age out of care in California with no place to call home

  • Half of youth who have aged out of foster care end up homeless or incarcerated

Transitioning out of the foster system can be challenging, especially for youth who have grown up in the child welfare system. They are expected to find housing, get a job, research and enroll in post-secondary or vocational educational opportunities, take care of their basic needs, and function independently as an adult.

Our one-on-one mobile case management helps these former foster youth with this transition by bringing support and advocacy to them wherever they are. C/FORWARD staff work with students and transitional aged youth on an individual basis to ensure they have access to housing, educational opportunities, legal services and employment.  C/FORWARD clients also will join our C/HOPE clients attending workshops, classes, field trips and special events.
